    While stationed at CGAS St. Petersburg, Florida, LCDR Lewis died returning from a SAR mission in which his crew rescued four survivors and were transporting them back when the helicopter crashed into the Gulf of Mexico. (Download pdf)

    LTJG Brostrom was the commanding officer of the cutter Point Welcome during a tour of duty in the Republic of Vietnam. In a tragic “friendly fire” incident, several U.S. Air Force aircraft mistakenly attacked the Point Welcome while she was on patrol during the night. (Download pdf)

    Killed in Aircraft Ops on November 26, 1969

    LTJG Clark’s HH-52A helicopter crashed into the Gulf of Mexico while searching for two missing fishermen. About 25 minutes into the search the crew broadcasted a “mayday”; no further transmissions were received. (Download pdf)
